File an Insurance Complaint

A large part of the Department of Insurance, Securities and Banking's mission is to protect the interest of consumers with objective, accurate, and useful information to assist them with intelligent insurance decisions. The Consumer and Professional Services Division investigates and responds within 45 days to requests from consumers for assistance in resolving disputes, including claim denial, misrepresentation, or policy cancellations. For further assistance, please contact the Manager of Consumer Services at (202) 727-8000.

To file an insurance complaint online:

    1)    Select Insurance Complaint Form and complete all of the fields.
    2)    Select the Submit button to automatically file the form.

To file a hard copy insurance complaint:

    1)    Select the Insurance Complaint Form and complete all of the fields.
    2)    Select the Print icon to print a hard copy.
    3)    File by fax at (202) 354-1084, or mail to:
